Erik ten Hag believes his team must look in the mirror at themselves after another mistake saw them drop points in their 1-1 draw with Burnley.
United had to wait until the 79th minute to open the scoring, when Antony put the home side in the lead, which looked like putting Ten Hag's side on the road to three points.
However, three minutes from time Andre Onana mistimed his jump for the ball and clattered Zeki Amdouni to give away a penalty.
Amdouni dusted himself off to take the spot kick and ensure Vincent Kompany's men earned a share of the spoils at Old Trafford.
After the match, Ten Hag was asked if United's recent mistakes were down to bad luck or mentality, admitting it was a combination of both, and that his team need to take responsibility.
'Both - bad luck and we are not lucky with the refereeing in recent weeks,' the Dutchman said.
'We've conceded so many penalties, this one is justified but other ones. If you make [Aaron] Wan-Bissaka in the final a penalty then what is this [Alejandro Garnacho penalty claim]. Where is the consistency?
'We have to look in the mirror. We made a mistake, but you take responsibility and they should understand what it means for a team and for a manager.'
The game was full of chances for both sides, but remained goalless until the final stages.
Tan Hag was pleased with his team's performance, but said he was frustrated they gave it away at the end.
Adding: 'Strong start to the game. Loads of chances then we lost in control in second part of first half. Burnley came back in the game and had some chances. Second half was all ours and we are in a winning position and we give the game away at the end.
'Very frustrated, shouldn't happen. You put yourself in difficult positions. Over many phases of the game we did very good so at the end we have to take the three points and the clean sheet. We gave it away again at the end.
'We put ourselves in a winning position by playing some good football - creating loads of chances and then to give it away in the end in the final minutes it's so unnecessary.'
The draw means the Red Devils are now 12 points off fourth spot, meaning their hopes of European football next season rely on finishing in a Europa League spot.
'That is the facts,' Ten Hag finished by saying.
'It is not easy to get fourth when you're so far behind. We needed to win. The past few weeks we've got in winning positions and then given it away. The gap is too big in this phase of the season.'

It’s really frustrating… Thy easily give penalties against us but if it’s for us they wave it off.,. We only get penalties in instances where it’s not debatable at all…

First you, Which coach replaces a striker when you intend to score & win. MAINOO was right call, no doubt, but not Hojlund. He should have replaced Bissaka with Amrabat & just tweak. formation to 3-5-2. Innovative ideas makes a coach successful
