The young Spaniard appeared to suffer a muscular problem on two occasions in extra-time, dropping to the floor each time, and that caused Sarri to decide that he should bring on Caballero.
Kepa then ignored his coach by refusing to come off, which led to Sarri - and the Chelsea coaching staff - looking infuriated as their player had showed a complete lack of respect.
Sarri even went to walk down the tunnel and leave the game prematurely, but he made the choice to stay.

Raheem Sterling hit the winning penalty as Manchester City retained their EFL Cup crown with a 4-3 penalty shoot-out victory over Chelsea following a goalless draw that saw Kepa Arrizabalaga incredibly defy under-pressure Maurizio Sarri.
City hammered Chelsea 6-0 at the Etihad Stadium two weeks ago but Maurizio Sarri's switch of system to use Eden Hazard as a false nine โ with Gonzalo Higuain missing out at Wembley on Sunday โ boosted his side's defensive solidity.
Pep Guardiola claimed pre-match it was too early to talk about City's chances of winning a historic quadruple despite being in the FA Cup quarter-finals, leading their last-16 Champions League tie with Schalke and sitting neck-and-neck with Liverpool in the Premier League.
Remarkably, an injured Kepa would not make way for Willy Caballero in extra time and, although he saved one spot-kick, Sterling converted the decisive effort for City to deny Sarri, reportedly close to the sack, his first trophy as a manager.

Can we just take a minute and talk about Jorginho. How can he be in starting XI and even bigger question - What was that penalty?!?! miracle that he hit the ball or not to break his leg. Should have given it to Odoi. Chelsea need to bring back some loanees and sort their team out. Oh, and the Kepa controversity - I don't think he was wrong not wanting to get off of the field. He was already warmed up and in this case the only time you do a goalkeeper sub would be when it is predecided that another keeper would be tasked with studying the penalties, leaving the main one to focus on the game. Kepa is a complete fool. This is the highest degree of disrespect to a manager. Finally, he lost the match for Chelsea because he reduced the team's morale by that action. I hope he'll be fined by the club. So self-centered.
