Gary Neville is increasingly worried about the future of Ruben Amorim, claiming that he has 'seen this film' before when it comes to underperforming Manchester United managers.
In front of minority owner Sir Jim Ratcliffe, CEO Omar Berrada and Director of Football Jason Wilcox, Amorim could only look on as his United side got torn apart by Manchester City, eventually going down 3-0.
It means that Amorim has managed only eight wins from 31 Premier League games and three of those teams are now playing in the Championship.
'Man Utd are 14th, we're only four games in,' Neville said on Sky Sports.
'We can't be getting into October where Man Utd are 14th or 15th in the league otherwise the manager will be in trouble. They have to start winning quickly.
'Next week if they lose [against Chelsea], they would go to 15th and 16th. It's five games in and before you know it you're in October and they're in the bottom half of the table. And that's where they can't be, having spent £200million and giving him a pre-season.
'There's got to be a turnaround pretty quickly, and the manager's idea has got to land very quickly with the players.'
Amorim insisted after the game that he will never compromise on his system or his philosophy and that ownership would need to remove him from his job if they desire a change.
For Neville, even four games into the new season, the writing appears to be on the wall.
'I'm worried about the manager, I'm worried about what's going to happen in this next few weeks,' he added.
'I don't think it's a time for panic, but I've seen this before, we've seen this film.'
United continue to rewrite the record books with every passing defeat and this was another new low.
They have also just picked up just 31 points from 31 league games since Amorim took charge which is the worst record of any ever-present Premier League team in that time.
Tottenham Hotspur, who finished 17th last season and beat United in the Europa League final to secure Champions League football, have improved dramatically this season under new boss Thomas Frank to add further scrutiny to how long it is taking Amorim to get things right.
United's latest failing also means that Amorim has the worst win percentage of any permanent Red Devils manager since the Second World War at 36.17 per cent.
But former Manchester United defender Danny Simpson is not pinning all of the problems on the manager.
He has instead pointed the finger at three players he believes 'do not work in the Premier League' and cannot be used moving forward by Amorim.
'The ball goes to [Patrick] Dorgu on the left. City let him have it [because they know] he's not going to hurt you,' Simpson told Stadium Astro on Sunday.
'He'll try and run up and down all day, but we've got nothing down the flanks.
'When Amad [Diallo] plays as a wing-back he's going to get it, he's going to do something.
'But when you've got Dorgu or [Noussair] Mazraoui or [Diogo] Dalot, I just don't think that can work in the Premier League.
'And trying to create chances… I felt sorry for [Benjamin] Sesko today. He's trying but there's nothing he can do.'
Ratcliffe was seen burying his head in his hands as things went from bad to worse for United.
They have spent north of £200million on players and yet look as suspect defensively as they ever have since Amorim arrived.
'This United team lack that real quality. They struggle defensively. There are obviously issues,' Roy Keane added on Sky.
'You talk about the system but the manager is sticking to his guns he is not budging.
'You look at the results and the points per game, goals for and against. It is not good reading. It does concern me.
'As soon as the game starts you are worried but you get that feeling against any decent team. They have scored four goals and two of them were penalties. So they are not a free-scoring team. It is almost the same problems as last year.
'We were told the manager needed a pre-season, his own players and they have spent over £200m so we keep making excuses. When are we going to see signs?
'You see [Arne] Slot going into Liverpool, they end up winning the league and Frank to Spurs, are they improving? Yeah.'
But the biggest concern of all for Keane is increasing levels of doubt as to whether players are still buying into Amorim's methods given it has not gleaned positive results thus far.
'My worry is... maybe the players haven't got that much faith in it,' the ex-United hardman added.
'As much as the manager is saying 'this is where we are going', as the results don't come and you are getting beaten by the better teams, he is making loads of changes... it is like they are going round in circles.
'We gave him the benefit of the doubt when he first came in.'

Danny Simpson hit the nail on the head. From all the players that occupy that winged back position, Amad is the only one that offers any real threat when he gets on the ball. The others (Dorgu, Dalot, Mazraoui) just run up and down the flanks offering nothing. They hardly even give good crosses into the box which makes Amorim's system a waste of time and frustrates any centre forward. Bruno is forced to sit deeper and the team loses his crosses, key passes and shooting prowels. It's not just working right now.
