A spirited second-half performance from Aston Villa helped Steven Gerrard survive a scare at League One Bolton.
Trailing to Dion Charles's opener, Villa were level at the break thanks to Douglas Luiz before Danny Ings won a contentious penalty in the 63rd minute.
The striker looked offside as he raced through on goal, before shooting woefully wide and colliding with keeper Joel Dixon.
Ings stepped up to convert the spot kick and then turned creator. His ball found Lucas Digne, who beat Dixon with a smart finish, and Leon Bailey scored late to make it 4-1.
Former Villa boss Dean Smith was left fuming as his Norwich side were knocked out 5-3 on penalties by Bournemouth. Norwich were leading 2-1 but were pegged back by Brooklyn Genesini's 92nd-minute strike.
Goals from Raul Jimenez — who donned an eye patch and a pirate hook in his celebration — and Adama Traore gave Wolves a nervy 2-1 win at Preston.
There were no such problems for Nottingham Forest, who eased past League Two Grimsby with a 3-0 win. Ryan Yates hit the opener, with Sam Surridge scoring in both halves.
In the first ever meeting between Cambridge and Southampton, the Saints advanced 3-0 thanks to a double from Che Adams, who netted his fourth in two games.
Brentford won 2-0 at Colchester, Keane Lewis-Potter scoring on his full debut with Mads Bech Sorensen adding a late second.
