Kelly joined Juve on-loan in January to the end of the season with an obligation to buy for £20m.
Juve have now triggered the option.
Kelly will formally sign for Juve on July 1 with Newcastle announcing the deal on Friday.
As yet, there is no word on the defender's length of contract at Juve.
