The Spaniard lingered after the full time whistle and made a point of showing his appreciation to the crowd.
He placed on hand on his chest and waved to those in the stands with the other in an apparently meaningful goodbye.
The 31-year-old has been heavily linked with an imminent move to Thierry Henry's Monaco side in Ligue 1.
Fabregas has been used sparingly by Maurizio Sarri so far this season and came of the bench against Southampton.
He replaced Ross Barkley in the 68th minute but failed to help his side break the deadlock in what could be his final fixture for the west London club.
Fabregas' current deal expires at the end of this season and Henry, who played with the midfielder at Arsenal, is keen to lure him to the principality.
Monaco are 19th in Ligue 1 and desperately need points to flee the relegation zone.
No doubt Fabregas' experience would be a huge boost but Sarri is reluctant to let him go.
He said: 'In this position we have only Jorginho and Fabregas and it would be a very big problem to be without Cesc. I want him to stay.
'If Cesc will go, then I think we need to buy another player, and it's not easy because technically Cesc is a very important player, and it's very difficult to find in the market a player with these characteristics.'
