On this day in 2015, Hazard secured EPL title for Chelsea

The victory was secured by Eden Hazard's goal on the stroke of half-time, the Belgian heading past Julian Speroni after the Palace goalkeeper had saved the PFA Player of the Year's penalty.

That gave Mourinho his third title as Chelsea manager and the first of his second spell in charge. It is the club's fifth top-flight title and comes five years after their last and 60 years since their first.

The celebrations that swept around Stamford Bridge at the final whistle were fully deserved for a Chelsea side who have set the pace since the start of the season.

On this day in 2005, Liverpool beat Chelsea to reach UCL final in Istanbul and you know the rest...

Just four minutes into the game Liverpool managed to break forward thanks to a run from John Arne Riise. He found Steven Gerrard in the middle, who flicked the ball over the defence and into the path of the oncoming Milan Baros.

The Czech striker was clattered by countryman Petr Cech in the Chelsea goal, but rather than blowing for a penalty the referee allowed play to continue and Spanish forward Luis Garcia pounced to finish off the move.

But it wasn't a net-busting finish that would have befitted the sweeping attack, instead, it was a gentle poke that the officiating team judged to have crossed the line.

The Reds ended Chelsea's hopes of winning the treble that season, and secured their pathway to the final of a European Cup final for the first time in 20 years.

The rest, as they say, is history.

On this day in 1998, Man City relegated to England third division

It was their second relegation in three years, as City had just dropped from the Premier League in 1996.

City's battle for Division One survival went down to 3 May 1998, the final day of the season. City started the day in the second bottom position with 45 points, just one point behind Port Vale, Portsmouth, and Stoke City. City defeated Stoke 5-2, but Port Vale and Portsmouth also won their matches, so that City finished third bottom, one point away from safety, and were relegated to Division Two.

Royle managed to improve City's fortunes, earning immediate promotion back to Division One in 1999 and then to the Premier League in 2000, but was sacked in 2001 as City slipped back down to Division One.

On this day in 1973, Ex-Barca defender Reiziger was born

After making a name for himself at Ajax, with whom he won one Champions League, he went on to represent four teams in as many countries, most notably Barcelona for which he played in 249 official games in seven years.

In 1996 he signed for Milan, but after a season marred by injury, he departed for Barcelona where he spent the following seven years, arriving at the same time as compatriot and former Ajax boss Louis van Gaal; though not an undisputed starter, he nonetheless featured heavily for the Catalans, amassing more than 200 overall appearances and helping the club to back-to-back La Liga titles.
