It looked like they were going to drop points, until a huge mistake Fiorentina goalkeeper Pietro Terracciano provided a massive opportunity.
Rafael Leao took advantage of the error to fire the ball into the back of the net to the delight of the home crowd.
Stefano Pioli's side are five points clear of second-place Inter, who face Udinese later on Sunday.
What's next for AC Millan
Millan have three games left, knowing that a victory in each of those matches will see them become champions for the first time since 2011.
They visit Verona next Sunday before hosting Atalanta one week later and travel to Sassuolo on the final day of the season on May 22.
